Once you have gotten your account activated and changed your username and password, you can login here https://www.yourhealthfile.com/portal/login.jsp and you will be asked to update your contact information, insurance and medical history.
You can always update this information later. Log back in and click the blue box in the left lower corner of the home page labeled “Review Medical Record”
There are 2 sections of this information we ask you to complete before your first appointment.
- Patient Account:
- Patient Information: Click blue Update Patient Information Link and complete all requested information
- Patient Insurance: Complete all insurance information. This is what we will use to file for coverage of your visit. Incomplete or inaccurate information here may cause payments denials and we will pass those charges on to you.
PLEASE NOTE: you are expected to update your insurance information on the portal and provide us a copy of your insurance card any time your coverage changes.
- Review Medical Record: Please complete each section of this medical history as completely as you are able. We will review this with you at your visit.
If you have documents you want to share, you can bring those with you to your first appointment and we will get them scanned into your record and returned to you.
